Smoking and physical activity both show up in the published research on adipose tissue and the cells inside it. This guide covers six human studies and where each stops. None of them tested banked tissue, and none links either habit to any outcome from adipose tissue banking.

**TLDR Published smoking adipose tissue research reports lower proliferation and altered secretions in fat-derived cells from smokers, a modest negative correlation between smoking level and viable cell counts in one 43 patient study, and more complications after fat grafting. Exercise studies report smaller fat cells and more capillaries in fresh biopsies. All of it is early, small, and separate from banking. Banking adipose tissue does not guarantee eligibility for, access to, or benefit from any future therapy.**

You smoke, or you used to. Or you have run for years. Either way, one question arrives fast: does what I do with my body change what a laboratory would find in my tissue?

Save My Fat has published more than two hundred educational articles on adipose tissue banking, the FDA framework, state stem cell laws, and the adipose-derived cell trial pipeline since February 2026. This one reports what the smoking adipose tissue studies and exercise studies say, limits included.

Every finding below comes from fresh tissue, cultured cells, or surgical outcomes. None of it measured tissue that had been frozen and stored. Read it as background on [fat health](https://savemyfat.com/adipose-tissue-why-fat-health-matters-for-future-use/), not as a prediction about your own sample.

## Smoking Adipose Tissue Research: What Happens to the Cells in Fat?

Three human studies report that cells from smokers’ fat behave differently from cells from non-smokers’ fat, in a dish or at processing. Each is small, none is an FDA review, and none measured stored tissue.

In Biology Open in 2024, [Salah and colleagues](https://pubmed.ncbi.nlm.nih.gov/39625294/) compared adipose-derived cells, called ADMSCs, from healthy smokers and non-smokers. The smokers’ cells showed reduced proliferation, fewer viable cells, a cell cycle delay, and upregulated oxidative stress genes. The abstract does not give donor numbers and reports no clinical or banking outcome.

In a 2025 pilot study in the Journal of Burn Care and Research, [Deville and colleagues](https://pubmed.ncbi.nlm.nih.gov/39820680/) studied adipose-derived stem cells from eight patients with major burns. Cells from current smokers secreted more IL-4, IL-10, TNF-alpha, IL-8, and IFN-gamma than cells from non-smokers or former smokers. The authors concluded only that smoking status “may alter the paracrine factors secreted from ADSCs.” The study measured secretions, not yield or viability.

In Aesthetic Plastic Surgery in 2021, [Andjelkov and colleagues](https://pubmed.ncbi.nlm.nih.gov/33083844/) processed fat from 43 liposuction patients into stromal vascular fraction, or SVF, with one specific device. Viable SVF cells per gram fell as smoking level rose, a modest negative correlation (rho of minus 0.312, P below 0.05). Age correlated positively with viable cells, and body mass index and gender did not influence yield or viability. Absolute yields are not in the abstract.

These are preliminary findings from small groups. SVF appears here as a research measure only, not a product or service, and no adipose-derived stem cell product is FDA approved for any condition. What the findings might mean for you is a question for a licensed physician.

## What Does Smoking Do to Fat Grafting Outcomes?

The fat grafting data report complications, not graft survival. In a 2022 multicenter database study in Aesthetic Surgery Journal, [Nguyen and colleagues](https://pubmed.ncbi.nlm.nih.gov/35299249/) reviewed 789 patients who had breast fat grafting. Smoking was an independent risk factor for any major complication within 45 days, with a relative risk of 17.1, and for infection, with a relative risk of 20.2.

This is a surgical outcome with fresh fat, so it says nothing about stored tissue. The study did not report graft retention, so it cannot tell you whether smokers’ fat takes less well.

Fat grafting is a surgical procedure, not a service connected to banking. These figures come from one database study, not an FDA review, and remain early evidence. If you are weighing any procedure, a licensed surgeon is the person to ask how smoking bears on it.

## What Does Exercise Do to Fat Tissue?

Two studies from the same research group report that exercise changes the structure of subcutaneous fat, even without weight loss. Neither measured stem cell yield or viability, and neither involved stored tissue.

In the Journal of Physiology in 2022, [Ahn and colleagues](https://pubmed.ncbi.nlm.nih.gov/35249225/) trained 36 adults with obesity for 12 weeks while their weight was maintained, with abdominal biopsies before and after. Training reduced fat cell size and raised capillary density. Most of the changes were no longer significant four days after the last session.

In Nature Metabolism in 2024, [the same group](https://pubmed.ncbi.nlm.nih.gov/39256590/) compared people who had done endurance exercise for more than two years with matched non-exercisers, all with overweight or obesity. The exercisers’ subcutaneous fat had higher capillary density, fewer macrophages, and more mitochondrial proteins. The abstract does not state the sample size, and a cross-sectional design cannot show cause.

One finding cuts against expectation. In the Andjelkov cohort, SVF cell viability correlated negatively with physical activity level (rho of minus 0.432, P below 0.01). It comes from a single device-specific cohort, so it is no basis for personal advice. These findings are preliminary and not FDA reviewed, and a licensed physician is the person to interpret them. Our post on [aging and inflammaging](https://savemyfat.com/adipose-derived-stem-cells-aging-and-inflammaging/) covers how age relates to fat tissue.

## How Do the Six Studies Compare?

The six studies are small, mostly laboratory or observational, and none measured banked tissue. The table lists each design, finding, and gap.

Study Design and size Main finding Not measured

Salah 2024 In vitro, donor count not in abstract Smokers’ cells proliferated less Clinical or banking outcome

Deville 2025 Pilot, eight burn patients Smokers’ cells secreted more cytokines Yield or viability

Andjelkov 2021 43 liposuction patients, one device Smoking level correlated negatively with viable cells Stored tissue

Nguyen 2022 Database, 789 breast grafting patients Smoking raised complication risk Graft retention

Ahn 2022 36 adults, 12 weeks, weight maintained Smaller fat cells, more capillaries Stem cell yield

Ahn 2024 Cross-sectional, size not in abstract More capillaries, fewer macrophages Cell yield, causation

Read down the last column: no study links smoking or exercise to any outcome from stored adipose tissue.

## What Does None of This Research Show?

None of these studies shows that smoking or exercise changes banked adipose tissue, because none of them studied banked tissue.

No study shows that quitting smoking raises cell yield or that starting exercise raises viability. The smoking data are small-sample correlations and cell behavior in dishes. The exercise data describe tissue structure in fresh biopsies. Groups of eight, 36, and 43 do not support personal predictions.

The research also does not show that these cell measures matter clinically. Proliferation, cytokine secretion, capillary density, and viable cells per gram are laboratory endpoints. Banking preserves your own tissue, and any future use depends on FDA regulatory status, physician guidance, and the pathways available at that time.

## What Can a Patient Do With This Information?

Talk to your physician, and tell both the bank and the physician who would perform any harvest about your smoking status.

This post makes no statement about what any laboratory, including our laboratory partner, measures or records. What you can do is ask a bank, in writing, how it documents donor history, what it reports back after processing, and what its storage agreement says about those records. Our guide on [banking age](https://savemyfat.com/what-age-is-best-to-bank-adipose-tissue/) works through similar questions about timing.

Whether quitting before a harvest changes anything measurable in your tissue has not been studied, and this post makes no promise that it does. Our page on [healthy aging](https://savemyfat.com/wellness-and-healthy-aging/) covers general habits without tying them to banking.

## Frequently Asked Questions

### Does smoking mean my banked fat will be unusable?

No study has tested that. The research shows differences in fresh tissue and cultured cells from smokers, in small samples. Nothing links smoking status to any outcome from stored tissue. Tell your physician and the bank your smoking history.

### Will quitting smoking or exercising before a harvest improve my cell yield?

No study has measured that, and this post makes no such claim. The one study that looked at physical activity found a negative correlation with viability. Talk to your physician about smoking and exercise for your health in general.

### Is any of this research FDA reviewed or part of an approved therapy?

No. These are early stage peer-reviewed studies, not FDA submissions. No adipose-derived stem cell product has FDA approval for any condition. Banking does not guarantee eligibility for any future trial or program, and a licensed physician is the right person to interpret these studies.

## Key Takeaways

Smoking adipose tissue research consists of three small studies: reduced proliferation in smokers’ cells in a dish, altered secretions in eight burn patients, and a modest negative correlation between smoking level and viable SVF cells in 43 liposuction patients. A 789 patient database study found smoking raised complication risk after breast fat grafting, without reporting graft retention. Two exercise studies found structural changes in fresh fat. No study measured banked tissue, and none shows that quitting or exercising changes what a bank would find. Tell your physician and the bank your smoking status.

Save My Fat connects patients and providers with a U.S.-based tissue bank. It is not a tissue bank, a laboratory, a medical practice, or a treatment provider. Stem cell and regenerative medicine regulations vary by state, including specific informed-consent and disclosure requirements in Florida, Utah, California, Georgia, and Tennessee governing tissue and stem cell services. Banking adipose tissue does not connect patients to any treatment pathway, and any future use depends on FDA regulatory status, physician guidance, and the availability of approved or investigational pathways at that time.

Readers weighing adipose tissue banking for potential future use can review [current pricing](https://savemyfat.com/pricing/) or bring their questions about donor history to the [contact page](https://savemyfat.com/contact-us/).
