## What the Phase Distribution Says About the Field

## Recruiting Is a Status, Not a Verdict

The [ClinicalTrials.gov glossary](https://clinicaltrials.gov/study-basics/glossary) defines recruiting in a single sentence: the study is currently recruiting participants. That is the entire definition. It says nothing about safety, nothing about benefit, and nothing about whether the study will finish.

ADIPOA2 shows what a completed study can say that an open one cannot. It randomized patients with mild to moderate knee osteoarthritis to a single injection into the joint of autologous adipose-derived mesenchymal stromal cells, at two million or ten million cells, or to placebo. Pers and colleagues published [the randomized adipose cell result](https://pubmed.ncbi.nlm.nih.gov/40885690/) in Annals of the Rheumatic Diseases in December 2025, with 97 patients analyzed.

At six months, 26 patients (47.3 percent) in the cell arm and 23 patients (54.8 percent) in the placebo arm were strict OARSI/OMERACT responders, a relative risk of 0.86, 95 percent confidence interval 0.58 to 1.28, P value.46. The authors concluded that a single injection did not significantly improve pain and function. The placebo arm did numerically better than the cell arm.

ADIPOA2 was recruiting once, and its record looked much like the rows above, in the same condition that occupies both late phase slots on this list. A list of open studies is an inventory of questions, not an evidence base.

## Reading the Condition Column Carefully

The condition column is what readers scan first, and it is the part most likely to mislead. A study existing in a condition is not evidence about that condition. It means a sponsor obtained clearance to ask a question and found a site willing to ask it.

FDA’s [consumer alert on regenerative products](https://www.fda.gov/vaccines-blood-biologics/consumers-biologics/consumer-alert-regenerative-medicine-products-including-stem-cells-and-exosomes) names arthritis and Alzheimer’s among the conditions these products are not approved for, alongside Parkinson’s, ALS, cardiovascular and lung disease, autism, blindness, and COVID-19. Two of those named conditions appear in the table above. Both things are true at once: a trial can be open, and no approved product exists.

## The List Reflects Late July 2026 and Nothing After

Trial status changes constantly, so this table will age.
